The CHI4Health application serves as a comprehensive tool for patients of Cincinnati Health Institute, providing access to personalized wellness plans. The platform includes features such as daily meal planning, exercise recommendations, lifestyle modification guidance, and motivational support.

Patients receive targeted content aligned with their individual care plans directly within the app. It facilitates tracking of various health metrics—including caloric intake, physical activities, body measurements, step count, hydration, and sleep patterns—through an interface that enables real-time sharing with healthcare providers. Furthermore, secure communication channels allow for ongoing interaction via personal journals and private messages.

Integration with Apple HealthKit enhances functionality by allowing users to share data such as steps and exercise duration seamlessly with the app.

The user interface is intentionally designed for ease of use by Cincinnati Health Institute patients exclusively. Usage is restricted to authorized individuals using iPhone or iPad devices.
